All Inclusive Elder Care (AIEC) Mission Statement: Providing quality preventative and primary health care services for the elderly in the Hillman community. Philosophy: The AIEC philosophy is to provide compassionate, quality, and preventative services for seniors in an effort to help maintain a healthy rural community. The nurses of AIEC provide holistic care to senior community members that encompasses their physical, emotional, and spiritual health. We strive to protect their dignity while providing the best care possible to help them maintain a healthy and meaningful life. As client advocates, AIEC partners with other health care providers in the community to ensure all our senior community members are living in safe environments and have access to quality health care.

Organizational Objectives To provide assessment of the physical and emotional wellbeing of the local elderly population, and facilitate the appropriate preventative care to detect early warning signs of illness to prevent hospitalization. To assess and care for greater than 25 local elderly clients within the first 6 months of operation. To have a 75% patient satisfaction rating on the services offered by the end of the first year.

Organizational Goals Provide information and services to promote healthy eating, exercising, and living to our elderly clients. Assess, promote, and facilitate the selection and use of appropriate preventative health services to our elderly clients. Utilize current evidence-based research and practice to guide the services and recommendations made to our clients.

Job Descriptions Executive Director Nurse Practitioner Oversee all staff and administrative personal Prepares and manages budget Delegates duties appropriately to AIEC staff Adhere to the Mission, Vision, and Philosophy of the AIEC Work with coordination of AIEC with local physicians for patient referrals Supervises staff to ensure the clinic operates in an efficient manner and patients are receiving quality care Assess clients’ physical and mental status, order necessary diagnostic labs and tests, and orders referrals as needed Develops, maintains, and educates staff on policies and procedures Provides educational opportunities for staff Arrange contracts with nursing students to volunteer Community outreach: Provide informational pamphlets at senior community centers Promote AIEC services and answer questions at public functions Arrange informational meetings with local health care providers to promote AIEC services Registered Nurses Adhere to the Mission, Vision, and Philosophy of the AIEC Assists Executive Director, NP with writing policy and procedures Reports any patient abnormalities to the Executive Director, NP Collaborates with the AICE team to evaluate and provide referrals for clients to community resources Obtain patients' health history, medication list, allergies, and document chief complaint Weigh patients and take vital signs Perform limited podiatry care Perform simple procedures such as urine dipsticks, finger stick glucose, phlebotomy, wound assessment and care Maintain accurate treatment records Oversee nursing students Maintain a clean and safe working environment Community outreach: Provide informational pamphlets at senior community centers Promote AIEC services and answer questions at public functions

Job Posting Registered Nurse Required All Inclusive Elder Care (AIEC) is seeking an experienced registered nurse for a full-time position in our new health clinic. We are looking for an enthusiastic nurse who will support our mission statement and provide compassionate, quality care to the patients in our rural community. AIEC Budget Proposal Fiscal Year 2014 Patient revenues: Cost for client $30 per visit flat rate. Visit length 30 minutes. Hours of operation (9a-5p, less 1 hour for lunch and 30 minutes for office preparation)= 6.5 hours daily. Appointment goal per day= 15. Utilizing 2 RNs each day will allow for overlapping of appointments to meet 15 clients per day goal. Grant income: AIEC will apply for grants from federal and local governments and organizations. AIEC serves the underserved and underfunded area of Hillman, MI. Expected annual grant funding= $115,000. Other operating revenue: $20,000 Funds from referral programs and local partnerships. Mortgage and Escrow: Facility cost $89,000. 30 year mortgage at 3%, plus escrow account= $5,400 yearly Utilities: Gas, electric, phone/internet, water/sewer, garbage, lawn and snow removal= $9,600 Supplies: Medical supplies, office supplies, advertising costs, cleaning supplies= $30,000 Salary Costs: Executive Director Nurse Practitioner annual salary= $62,400 ($20/40 hour week), Registered Nurse annual salary= $35,000 ($18/ 40 hour week) X 2 FTE= $70,000 Office Manager annual salary= $31,200 ($15/ 40 hour week) House Keeper annual salary= $9,880 ($9.50/ 20 hour week) Full time employees (4) $5,000 annual fringe benefits= 5 paid holidays, paid lunch, paid time off, medical benefits. Excess of revenue= $2,230. Actual excess of revenue will be allocated toward a hardship fund for clients who cannot afford the full office visit cost.
