FILES Processing Pilot & Flight Instructor Certification Files
During the 2006 FY, the files received by AFS-760, Airmen Records in OKC was 158,893 Pilots and 56,606 CFIs. The # of pilot and CFI files returned: ATP , Sport Pilots Commercial Pilot-2, Student Pilots Total Pilots 6% Private Pilot , Sub Total---9,534 Recreational Pilots CFI’s , Total CFI’s 5% Per AFS-760 on Total , Total Pilots & CFI’s 6% DPE awareness and IACRA has contributed to improved performance. During the 2004 FY, the file return rate (files returned by AFS-760, Airman Records in OKC) stood at 5%. Per AFS-760 on During the 2006 FY the return rate stood at 6%. A slight increase. Per AFS-760 on 95+% OF ALL PRACTICAL TEST FOR PILOT CERTIFICATES AND RATINGS WERE ADMINISTERED BY PILOT EXAMINERS. THIS MEANS THAT PILOT EXAMINERS ARE THE BEST PLACE TO ADDRESS FILES ACCURACY!!!! <CLICK>

UNACCEPTABLE!!! FILES A file return rate of greater than
5 files within 12 months from your CO is UNACCEPTABLE!!! A FILE RETURN RATE GREATER THAN 5 FILES WITHIN 12 MONTHS IS UNACCEPTABLE! WHEN A FILE IS RETURNED BY THE FSDO OR AFS-760 APPLICANTS SOMETIMES DO NOT GET THEIR PERMANENT CERTIFICATES BEFORE THEIR TEMPORARY CERTIFICATES EXPIRE!!! THIS CAN CAUSE MANY PROBLEMS, ESPECIALLY IF THERE IS AN ACCIDENT OR INCIDENT WITH THE NEWLY CERTIFIED PILOT POSSESSING ONLY AN EXPIRED TEMPORARY CERTIFICATE. This training on reviewing and filling out the airman application (8710-1)should help improve this figure.

COMMERCIAL SOFTWARE The FAA has legal authority, under 49 U.S.C (a)(2) and 14 CFR Part 61.13(a) to prescribe the “form and the manner” for submitting application for the issuance of airman certificates. Commercial software must meet the following standards: 1. If the software program cannot ensure the application is printed on a single sheet of paper, (front and back) then applicants must provide their name, date of birth, and certificate number on the bottom right hand corner of page two. (SEE THE BOTTOM OF THE PRIVACY ACT STATEMENT TO FIND THIS INSTRUCTION). 2. The formatting, fonts, density and size must be identical to the FAA form in current use, and cannot be altered by any print process or computer system. 3. The software program must produce a form identical to the current FAA form. That is, comply with the Office of Management and Budget requirements. 4. The software program must be identified as a commercial form bearing the name and address of the company. <click>

XIV. CONDITIONS OF ISSUANCE This is an interim certificate issued subject to the approval of the Federal Aviation Administration pending issuance of a certificate of greater duration. It becomes void- 1. Upon the receipt of a certificate of greater duration to replace it; 2. Upon a finding by the FAA that an error has been made in its issuance; 3. Upon finding by the FAA that is was issued illegally or as the result of fraud or mis- representation; 4.
