12.  Midwife’s clinics are held at  Hillside Community Centre (Sunshine Group)  and patients will also be seen at  Acorns Children’s Centre, Violet Hill Road, Stowmarket MIDWIFE CLINICS

13.  The surgery will no longer be open for clinics or the collection of prescriptions on Saturday mornings from the 1 st of October 2015. SATURDAY MORNING

15. 16.  Informed implied or expressed consent will be obtained before any procedure or examination is carried out.  An explanation will be given to the patient sufficient to understand the procedure.  Should the procedure carry a risk that the patient is likely to consider as being substantial, expressed consent (written or verbal) will be obtained.
