Filling out the TAG Form …It’s now done electronically!

Why do classroom teachers fill out this form? Most of your TAG student’s instruction will take place in your classroom. By law, his/her instructional rate and level need to be met 100% of the time.

Areas of Giftedness Intellectually Academically (reading/math) Potential to Perform Whatever the “giftedness category,” DO fill out the ENTIRE form.

RATE of Learning Rate of Learning is a measure of the pace at which a student successfully progressed through the curriculum after being placed at the appropriate level. A student’s rate of learning will vary depending on the following: subject point in the learning process degree of interest level of difficulty learning style of the student

LEVEL of Learning Write the NAME of the assessment and its results in the chart. One assessment for each line on the chart. 3-4 total for reading, 2-3 for math, 1-2 for others. NOTE: I will obtain DIBELS, OAKS and STAR Reading scores.

Instruction Specific activities the student will be participating in that meet his/her RATE and LEVEL of learning. Intellectually gifted students: list activities for each subject area Math or reading giftedness: list activities especially geared towards these subjects. Be specific rather than general in your description of instructional activities.
