Case Management Fundamentals WHAT IS CASE MANAGEMENT? Case Management is a youth-centered goal-oriented process for assessing the strengths and needs of an individual for participant services; developing a plan to: Attain their goals Assist him/her to obtain the determined services Facilitating their use of services in obtaining their goals (education, goal-focused employment, i.e., long-term employment, etc) KEY COMPONENTS –Thought out written process –Staff who “get it” –Organized case files –Services to offer –Strategy to discuss youth goals, progress, challenges and next steps

Case Conference What is a case conference? Intentionally scheduled reoccurring meetings to formally review and discuss the goals of a youth. TIME ALL STAFF IDPDATA ACTION STEPS CC COMPONENTS

Case Conference Process Case Management facilitates CM: Shares objective data: attendance, progress toward IDP goals, etc. Go (2) Go-round: other staff share their data I see…. I think…. I believe the next step for the student is….. Strategy/ Commitments Case Manager recaps action steps and commitments Check-in point is established Suggested Time Frame: 5 – 10 minutes
