SETTING GOALS BE MEASURABLE BE FEASIBLE/DOABLE POSITIVE IN DIRECTION DEVELOPED WITH THE CLIENT TO REFLECT CLIENT’S DESIRES

Reduce Goal to Manageable Size The ultimate goal of a single mother is to regain custody of children removed from her custody because of neglect. Objectives: Improve conditions in the home by developing and following a house keeping schedule developed in collaboration with Family Advocate.

Improve parenting effectiveness by participating in a parenting class and in a parent’s group. Explore and develop support systems that will make child care available, thereby freeing time to socialize in the evenings.

Planned Services/Activities Ms. Doe will take out soiled pampers daily. Ms. Doe will wash dishes daily after each meal. Ms. Doe will wash clothes at apartment laundromat twice a week. Child Protective Caseworker will monitor completion of tasks monthly. Ms.Jones, Family Advocate will make weekly home visits to assist Ms. Doe in completing tasks for two months until a routine is established.

MONITOR PROGRESS BE SUPPORTIVE ENCOURAGE/REWARD STRESS BENEFITS OF COMPLETING TASKS TO ATTAIN GOAL DISCUSS AND RESOLVE BARRIERS TO SUCCESS

EVALUATE REVIEW EACH STEP TOGETHER TO SEE WHAT IS WORKING AND WHAT IS NOT IF ITS NOT WORKING DO SOMETHING DIFFERENT.

FINAL CLOSURE THE PARTNERSHIP ENDS WHEN THE CHILD LEAVES THE PROGRAM CLOSURE OCCURS WHEN SERVICES ARE RENDERED AND DOCUMENTED
