1 SDPI Healthy Heart Project Albuquerque Indian Health Center & Santa Ana Community Wellness Center & Santa Ana Health Center Presentation Albuquerque Service Unit

2 What is the Healthy Heart Project (HHP)?  The Healthy Heart Project (HHP) is a program that works to prevent heart disease in people with Diabetes.  Diabetes is a major risk factor for heart disease.  Heart disease, which affects the heart and blood vessels, is a major complication of diabetes and is also the #1 cause of death among American Indians and Alaskan Natives.

3 Studies to Reduce Risk of Heart Disease  Studies have shown that it is possible to reduce your risk of heart disease by: Controlling blood pressure, cholesterol and glucose levels. Stopping smoking Taking an aspirin a day. Losing weight through a healthier diet and physical activity.

4 Why you should join the HHP!  The Healthy Heart Project can help you in several ways: You will be assigned a case manager who will work with you and your provider to meet the goals you should have for blood pressure, cholesterol, blood sugar, weight and physical activity, and help you stop smoking (if you smoke). Your case manager will be your team-mate who will help you prioritize the goals you have.

5 More Benefits of Joining HHP  You will take classes about reducing heart disease risks and learn about managing diabetes.  You may be able to delay or prevent getting heart disease by participating in this project.  Services provided for in project are FREE! There is no cost to you for participating.  You can even invite a friend to participate with you as your buddy.

6 What do I have to do when I’m in the Project?  Have a physical exam, lab tests, and an electrocardiogram (ECG) so we can learn more about your health.  Complete a baseline questionnaire that has to do with your physical activity, diet, your health history, health status and your feelings about the project.  Attend monthly case management meetings with your case manager.  Attend Honoring the Gift of Heart health classes.  Set goals for yourself that the Healthy Heart Project team members can help you accomplish.

7 What are the Goals?  Weight loss.  Increased physical activity.  Control heart disease risk factors with lifestyle changes and medications.  Learn how to make healthier choices about your nutrition and exercise.  Feel Better!  All services and classes will be offered in Santa Ana

8 HGHH Classes  Classes are taught using the Honoring the Gift of Heart Health Curriculum. There are 10 sessions and can be taught in group visits or in individual case management sessions.  Classes cover various topics in heart disease prevention education. Act in time to heart attack signs. What you need to know about high blood pressure, salt, sodium What you need to know about high blood cholesterol And more!

9 HGHH Classes  A graduation is held for participants in taking part in all 10 sessions and congratulations for making the effort to learn how to lessen the chance of heart disease and stroke.

10 Healthy Heart Project Events for Participants  Participants are invited to a variety of events. Walking Health Fair in Santa Ana American Heart Month Red Dress Campaigns Holiday Fest Taking Control of Your Diabetes Conference Healthy Heart Exercise Challenge Competition

11 What is the Eligibility Criteria?  To join the HHP, interested participants must meet the following… Native Americans 18 years and older who have type 2 diabetes. Must have a primary care provider at I.H.S. No alcohol or substance abuse issues. Not on dialysis. Not actively undergoing treatment for cancer. Not pregnant Willing to meet with your case manager on a monthly basis.
