2. Crane Incident/ Near Miss Incident: Incident: –A contractor employee was using a ‘cherry picker’ crane to set mats in preparation for an outage on a pipeline. The outriggers were extended only slightly at the beginning of the job in order to keep them on the mat. As the job progressed, and mats were being placed farther and farther away from the crane, it was not repositioned and the outriggers were not further extended. As the 11th mat was being placed, the crane overbalanced and tipped onto an Exxon propylene line, causing damage to 2 valves. Damage: Damage: –Extensive damage to valves and pipeline –Some damage to cherry picker –Deteriorated relationship with company that owns damaged lines –Community was affected- road nearby shut down Potential: Potential: –Major release of flammable gas –Injury Learning: Learning: –Ensure equipment operators are properly qualified. –Keep your eyes and ears open- do frequent observations, ask lots of questions and don’t be afraid to intervene!

3. Vehicle/ Equipment Damage Incident: Incident: –While covering a trench with a 330 track hoe operator hit a tree with the right side of the hoe. Damage: Damage: –Right side of 330 track hoe was damaged- dented, compartment door was crushed and sprung open, filter housing broken, other damage possible. Potential: Potential: –Injury –More extensive equipment damage Learning: Learning: –We must be diligent and use appropriate number of spotters- as many as needed. We often work in very congested areas. –Be sure to recognize and mitigate potential hazards such as this- STAC!

4. Vehicle/ Equipment Damage Incident: Incident: –While carrying a rack of drill stems a contract employee was going between a mud truck and a water tanker. A pick-up truck was parked on his blind side. When the employee started to swing the rack of stems away from the bore section, his counter weight came in to contact with the open drivers door of the foreman's pick-up truck. There was a spotter with the track hoe at the time of this incident, but he could not see the pick-up truck- his view was obstructed by the track hoe. Damage: Damage: –Left side door and fender Potential: Potential: –Injury –More extensive equipment damage Learning: Learning: –We must be diligent and use appropriate number of spotters- as many as needed. We often work in very congested areas. –Vehicles or other equipment not integral to current work should be parked well away –Be sure to recognize and mitigate potential hazards such as this- STAC!

Injury Incident: Incident: –An employee was injured when he slipped in the mud causing his head to come in contact with a valve wrench he was using Damage: Damage: –Cut to forehead (minor- ‘butterfly’ was needed) Potential: Potential: –More severe injury Learning: Learning: –Be sure to recognize and mitigate potential hazards such as this- STAC!

6. Injury Incident: Incident: –A contract employee and a co-worker were trying to move a full abrasive blasting pot. When they leaned the abrasive pot back, he lost his grip causing his hand to be pinched between the pot handle and a pallet of abrasive that was next to it. Damage: Damage: – Wrist pain (pinch) Potential: Potential: – More severe injury Learning: Learning: –The blast pot was too heavy to be moved without the use of a forklift or other lifting equipment –Recognize hazards and DO NOT TAKE SHORTCUTS!!

7. Injury Incident: Incident: –A contract employee felt eye irritation and after a period of time, he noticed the irritation continued which indicated he had something in his eye. Proper medical attention was received and particle was removed. Damage: Damage: – Eye irritation Potential: Potential: –Corneal abrasion Learning: Learning: –RCI Pending
