4 Traveler Monitoring Form Used by DPH to establish traveler record and create online account Detailed data related to travel and risk is captured Online account is established and login email is sent to traveler

6 Ebola Active Monitoring Used by DPH to track symptoms and compliance of travelers being actively monitored

7 Ebola Active Monitoring Query Used by hospital to identify if patients being treated are being monitored Hospital partner logs in and enters basic patient information Records matching search criteria are returned providing limited information on monitoring period

12 ADDITIONAL SPIN-OFFs Development of peer relationship among hospitals of same tier for sharing of best practices Designated hospitals will be required to participate in regular Ebola exercises to reinforce infection prevention activities and practices
