5. Self-Placement You MUST have prior approval from me before you self-place. These forms are not currently online. You MAY NOT self-place in a school with your relatives or children. You WILL be pulled from Internship when we find out. You may not self-place in a school classified as a priority school or in school improvement status as determined by ADE. I will call to verify your placements.

6. You MAY NOT Self-Place in: Conway Morrilton Greenbrier Little Rock North Little Rock Mayflower Benton Bryant Cabot Pulaski County Special School District Vilonia Fort Smith

7. Standard-Placement I will place you in a school. NO REQUESTS. You will be placed based on availability. Once you are placed, it will not be changed. You can be placed anywhere within your certification area. You will not be placed in a school more than 1 hour drive-time away. You will NOT be placed in Conway.

8. Standard-Placement Forms http://uca.edu/mat/internship/ All must be turned in to Nancy Ringgold in the Teaching and Learning suite by 4pm Friday, November 13th. If mailing, they must be postmarked by that date. NO EXCEPTIONS.

9. Request for MAT Internship Placement Application for MAT 6699 Intent to InternARIdeas Certificates Mini ResumePersonal NarrativeReceipt for $250 payment ALL MUST BE COMPLETE. WE WILL NOT ACCEPT BITS AND PIECES. DO NOT E-MAIL. HARD COPY ONLY POSTMARKED BY November 13th.

10. FBI/Arkansas State Police Child Maltreatment Registry You MUST complete both. Many places to do fingerprints Background Checks

11. ASP/FBI – Fingerprints We suggest LiveScan ADE or Local Co-op 2-3 days processing time http://www.arkansased.gov/divisi ons/human-resources-educator- effectiveness-and- licensure/educator-licensure- unit/background-check- requirements

12. Background Checks Otherwise – Local police stations, UCA, state police Takes longer – Up to 30 days Child Maltreatment Registry – Fill out, get notarized, mail in with payment

13. Background Checks Must be less than 1 year old for licensure Your responsibility to get them submitted on time and follow-up if needed Placement cannot be initiated until background checks are CLEAR https://adeaels.arkansas.gov/AelsWeb/ Search.aspx

14. FEES $250 Placement Fee Payable to the Cashier. Please let them know this is the Teacher Ed Candidate Admission Fee. This pays for your mentor teacher. You MUST pay if doing self OR standard placement. NO EXCEPTIONS. If you get a job, you WILL get a refund. Please allow 4-6 weeks for processing. Once you have a refund being processed, there’s no going back – until the following semester.

15. What Next?? I will e-mail to let you know your packet is received and complete. I will e-mail to let you know it’s received but something is missing. I will e-mail you your placement. CHECK YOUR UCA CUB ACCOUNT E-MAIL.

16. EMAIL I will e-mail you assignments to the email address on your application Initial e-mails to ALL interns go to UCA cub account I will create a listserv with your CHOSEN/PREFERRED e-mail address after that time

17. Please note… I WILL e-mail you your placement in late December I will NOT do a placement if your application is not COMPLETE. ALL PRAXIS MUST BE PASSED and submitted with your application in order to be placed.

18. Arkansas IDEAS http://ideas.aetn.org/ MUST complete Child Maltreatment Training Suicide Prevention Training Parental Involvement Training Dyslexia MUST print and submit a certificate of completion with your application.

19. Emergencies Happen…. We can HOLD your packet for ONE semester. You MUST resubmit a SIGNED advising sheet. We will ONLY hold paperwork for ONE semester.

20. Requirements You begin the FIRST day of YOUR school’s semester – NOT UCA’s. Starting late means extending your semester which could delay graduation It is YOUR responsibility to find out the start date. You go where your teacher goes – ALL DAY EVERY DAY.

21. Requirements Includes: Parent-Teacher conferences, required professional development days, work days, staff meetings, etc. There are 3-4 face to face meetings on campus during the semester. You will be notified of these dates ASAP
