What is a Coaching Call? 4

To provide you and your teams the important opportunity to: o Review performance data o Discuss barriers o Share successes o Ask questions o Encourage facilities to mentor one another An assigned faculty coach and HRET advisor will participate on these calls and support their development. Facility team leads are required to attend; however any and all team or staff member attendance is encouraged. Purpose of the Coaching Calls 5

Expectations, Roles, & Responsibilities Use online meeting technology or conference calls to host monthly meetings (e.g. WebEx, GoToMeeting, Adobe Connect) Communicate and confirm facility team lead participation in meetings Prepare for and lead the meetings (agenda, slide presentation, follow-up) Identify relevant topics for discussion Discuss data submission and identify trends in reports Troubleshoot, answer questions Highlight facility successes and help identify barriers Facilitate teams’ access to project resources Keep teams on track with project deliverables 6 Organizational Leads

Expectations, Roles, & Responsibilities Serve as the team expert resource providing direction and guidance Collaborate with Org Lead(s) in identifying and overcoming team barriers Engage in group discussions Review facility reports and create action plans for ongoing issues Participate in planning calls with the Org Lead(s) to help plan agendas Bring back relevant tools or latest research/information from the project’s Advisory Council meetings and share on coaching calls Notify Org Lead(s) if unable to attend coaching call and try to reschedule or find a fill-in coach for that month Faculty Coaches 7

Expectations, Roles, & Responsibilities Attend coaching calls Promote team participation in calls Share facility successes, tips, and ideas you’ve used to facilitate participation from your facility Speak up about facility barriers in the project and ask for help on behalf of the team 10 Facility Team Leads

Engage Facility Team Leads in Calls Circulate each month’s agenda well in advance Keep in touch regarding scheduling changes, please do not cancel calls without contacting your HRET Advisor (remember to also notify your faculty coach!) Listen to your facility’s unit’s learning needs and create tailored learning opportunities Consider asking some of your leads to present on calls ahead of time; facilities love to hear from each other Call Participation is Required Set expectations Follow-up with those unable to attend Maintain open communication about topic interests Leave time in presentation for questions, pose questions Communications Plan for Org Leads 11

Data collection, reporting and progress monitoring support will come from HRET and includes tools such as: Review Data Collection Onboarding Webinar #3 (ltcsafety.org) Comprehensive Data System (CDS) User’s Guide (ltcsafety.org) Review outcome data by pulling data from CDS Catheter Utilization CAUTI rates # of urine cultures ordered Review survey progress by pulling data from Cvent Facility Demographics Assessment Nursing Home Survey on Patient Safety Culture Skills Questionnaire Team Communication Guide Data Collection 12
