MOTIVATION ~8% of hospitalized patients are potentially dischargeable but they end up staying in the hospital for different reasons [ Lenzi et al. 2014 BMC Health Services Research ] Frailty index of elderly increases after 12 days of hospitalization Inefficient occupation of hospital beds, increases hospital costs (~1000€/day)
