5 ACCESS for ELLs® PRE-ID LABELS 5 OAA:DSR:CW 10/15/15

6 Pre-ID Label Use the pre-id labels for both online and paper/pencil testing. These are used for test booklets that need scoring. Do not complete the demographic information if using a pre-id label. Do not remove labels once applied because it will damage the test booklet, just apply another label that is appropriate. Do not write on labels. If the label has incorrect information, still use and correct the student’s information in WIDA AMS. If accommodations were used during online testing, mark the accommodation in the WIDA AMS. OAA:DSR:CW 10/15/15 6

7 District/School Label Use this label if a student is new and does not have a pre-id label. You will need to complete the demographic information on the test booklet. This label is used for test booklets to be scored. Use this label when issuing a new test booklet that is damaged, torn, or in multiple test booklet. Use this label if a student needs accommodations. OAA:DSR:CW 10/15/15 7

8 Do Not Process Label It is a white label with an orange stripe. Use this label for test booklets that are not to be scored. The test booklets with this label will be scored as blank even if there are responses. Do not put this label on test booklets that have not been used. A student writes in another student’s test booklet, transcribe into a new test booklet and affix the do not process one on the old one. If a label has been applied to the test booklet and the student moves, put this label on it. OAA:DSR:CW 10/15/15 8
