PRINCIPAIS C RESULTS 5 Aging and dependence carry with it, a host of other problems such as proliferation of chronic diseases and the onset of multiple comorbidities. The effects of demographic change and potentiate the global costs of Long term care; Portugal will be the 4th country in European Union with the highest percentage of elderly (31,9%); Decreasing in deaths and live births simultaneously (increment the dependence ratio of elderly). Between chronic disease and family burden The weight of demographic factors

6 New social facilities are need for strategic areas (mental health and palliative care); Skilled professionals/differentiation of care including a more diversified number of health professionals (reabilitations physicians, phsyiotherapists, social workers); Training to keep the guarantee quality standards. The balance between structure and human resources

7 It is important that the role of state in all its functions could focus on the Long term care/NHS continuing care over investment in the hospital sector Diversity of funding and proper mix of services( formal and informal) as the best prescription for future sustainability (Un) Sustainability of National Network - RNCCI

: 8 Monitoring at the base of continuing care supply; The prediction of the need of Continuous Care over times requires an investment in studies, surveys and audits that allow valid comparisons; It can be possible monitorize the evolution of dependent and vulnerable people (eg. Cancer and chronic diseases patients) Demand and Supply of RNCCI

9 The concerns about sustainnability of continuing care has been related to the diversity of funding and the appropriate combination of services by formal and informal caregivers To overcome the unpredictability of demand and supply of continuous care the importance of studies is emphasized. These are often supported by external evaluations that will redirect existing answers to create more adjusted measures. Conclusions
