1. Log into MISIS at MISIS.LAUSD.NET/START Use you single sign on user name and password

2. YOU NEED ‘SECONDARY ATHLETIC DIRECTOR ACCESS’ Click on the ‘select’ link. If your status is inactive call the help desk at 213-241-5200

3. Search for student, Type in first name with a space then the last name, or any part of first or last name. First Last

4. Click on the name. First Last

5. Click on the black triangle in the middle of the page

6. Mouse over Miscellaneous tab and click on Athletics

7. Click on District Required Documents

8. Enter appropriate dates Physical date should be the actual date of the physical. To enter the same date in the first 10 boxes, enter date here and select enter key.

9. Click on Save button. Make sure you see Save Successful
