20 Significant Safety Achievements- 11/15 Follow-up regarding the 70-295 pressure reactor incident is on-going: –The search for pressure reactors in lab areas continues. –New Level 3 Work Activities were developed and approved for Parr Reactor use in 62-342 (Doeff) and 62-314 (Chen). –A new Work Activity was developed and approved for a high temperature tube furnace in 70- 299 (Cairns). Safety stand downs and management walkthroughs were completed for all technical areas in ESDR, BTUS, and EAEI divisions. ETA lab areas participated in the recent DOE chemical safety assessment project. Lab areas visited included 70-269, 70-226, 70-173, and 62-316. The following ETA Safety Alerts were issued to assist lab areas in preparation for the planned walkthroughs: –Chemical Storage in Refrigerators –Hot Plate Safety –Electrical Equipment NRTL Requirements –Chemical Management System (CMS) Inventory Requirements Corrective actions for the recent static eliminator RPG registration issue were completed and closed-out.

21 Short Term Safety Outlook 12/15 Provide follow-up regarding the 70-295 pressure reactor incident as required. The incident review committee report with findings will be issued shortly. Provide follow-up on the corrective actions identified during the recent division stand downs. This mainly involves disposal of old or defective equipment. Continue to address ETA equipment on the electrical equipment “conditionally authorized” and “failed” lists. Ensure the 62-316 furnace hood is safe to operate for planned research using hydrogen. Complete the final 8 Work Activities pending in EHS collaboration. These are for B33 JCESR. Complete CATS items identified from glove box self-assessment project. Conduct a quarterly SAA hazardous waste inspection of ETA areas. Develop standardized spill cleanup kits for ETA lab areas.
