4 Identified processes that needed to be altered prior to implementation New Clinical Respiratory Scoring Tool Discharge checklist Updating Home Management, Plan of Care, etc. Implementation

5 Working with Quality to incorporate CHAT outcome measures Identifying key players to help in data collection Metrics / Data Management
