4 New Children’s Centre Pathway; 0 – 2 Years Programme Midwives Antenatal classes Ante-natal New Birth Visit by Health Visitors (HVs) CC Registration Baby feeding Groups (CBFT) By 2 weeks after birth Coping with Crying (HVs) Sleep clinics (HVs) - TBC By 8 weeks Little Connections post natal group 4 week (HVs, FSWs)- - Bonding & Attachment Public Health: starting solids 1 – 8 months Baby Massage - 5 week course 2 - 6 months Speech and Language stay & play: Babbling Babies Toddler Talk Play Time 8 months- 2 years Nursery education 2 years

6 Referral and Allocation Process Via e-CAF By phone Self-referral – ask family to go to nearest HUB Centre and ask to speak to a Family Support Worker  Case allocated (team nearest to where family live)  FSWs assess family and child’s needs (home visit)  Letter to network confirming allocation of case and name of FSW  At close of case - Outcomes Summary sent to network  Re-refer at any time
