All applications must be submitted through the GA’s office to the local MetLife Sales office. This will ensure the proper identifying forms are attached. All applications require a transmittal form (identifies the case as AIMCOR) All policies will be mailed to the GA’s office from your Met Sales office. 13 Individual Market MetLife offers three products in the Individual market The lifetime maximum benefit period for mental disorders and/or substance use disorders is limited to 24 months for all periods of disability combined. Transitional Your Occupation TPC Residual Disability Catastrophic Disability COLA 1-7% and COLA 3% Automatic Increase Benefit Social Insurance Offset Benefit Spousal Rider Guaranteed Insurability - An option to increase the benefit amount every year until the Insured reaches age 51, based on financial underwriting ONLY Lifetime - The Lifetime rider provides the Insured with a Total Disability benefit for the rest of the their life, during their continuous Total Disability, if the Insured becomes Totally Disabled before Age 65 and is still Totally Disabled at the first premium due date on or after the their 65th birthday Residual Disability - The Residual Rider provides partial benefits, under certain conditions, if the insured’s disability caused a 20% or more loss of earnings. Long-Term Care GPO - The LTC-GPO “locks in” the policyholder’s insurability, giving them the option to purchase an LTC policy at a future date without additional proof of medical insurability Catastrophic Disability - The Catastrophic Disability Rider will pay the Insured an additional amount of benefit (in addition to the base benefit) when the Insured is considered catastrophically disabled Catastrophic Disability means that due to injury or sickness, the Insured has a complete, irrecoverable and irreparable loss of: The use of both hands or both feet, or one hand and one foot or The sight in both eyes or Speech or Hearing in both ears or 6. Cost of Living Adjustment (COLA) - has a Built-in Buy-Up feature which allows the Insured to purchase any additional benefits accumulated through the rider after returning to work full-time following a claim which lasted at least one year The Insured must be under age 60 to be eligible to purchase the additional benefit and will pay an additional premium at the Insured’s then attained age for the increased amount of coverage 7. Automatic Increase Benefit – On each of the first five policy anniversaries, this option will automatically increase the insured’s base benefit by 5%. There is no cost to include the AIB to the policy. The additional cost will be associated at the time the increase takes effect. 8. Social Insurance Offset Benefit – This option will pay an additional benefit (after the EP) in the event the insured is not receiving any legislative benefit.
