JABSOM Annual Chair Reporting Instructions For Reporting Session: July 1 – June 30

Notes All reporting is for academic year (July 1 – June 30) Reporting is required for all compensated faculty only Faculty with joint appointments should complete a report for your primary department only You will need a UH email username/password to log in All text comments need to be kept at a 500 word limit Upon submission deadline, forms will not be accessible as to allow for the Dean to begin reviewing all entries.

Need a UH email account? UH email username/password is required to log- in to the reporting system If you have forgotten your username/password or need to obtain one go to the following website: UH ITS Email Set-Up UH ITS Email Set-UpUH ITS Email Set-UpUH ITS Email Set-Up Please note that your UH username/account will be issued directly by UH ITS Dept –Contact the UH ITS Help Desk at 956-8883 for assistance

To Begin Your Reporting Session Go to the Faculty Development Website: http://jabsom.hawaii.edu/faculty/facdev/annual-faculty-reports/ 1. Click on the “Annual Faculty Reports” tab 2. Click on “Annual Report Forms – All Faculty and Chairs Log in Here”

Log-In Screen You will then see the log-in screen. Log-in with your UH username and password (same as your UH email account)Log-in with your UH username and password (same as your UH email account)

1. Annual Chair Report Your contribution to your department and JABSOM’s annual report Used for annual feedback and to monitor progression towards applying for promotion and/or tenure Used towards UH annual faculty contract renewal process (as applicable) Be as accurate as possible When unsure, please make your best estimate Each year’s report will be stored for future reference

Reminders Click on the “save” button at the end of each section to save your work At the end of your session, you must click on the “submit” button to submit your form You may submit/change your data as many times until the deadline Upon clicking “submit” a printable version will appear Click on “Click to Print” button to print a copy for your files Click on “log out” to end your session If you need to make changes after submitting your report, please contact Lori Emery at lmatsuza@hawaii.edu or 692-0884 to re-open your report

1. Annual Faculty Report: Reminders Please be aware of the data being requested, in the block columns to the right, please indicate a numerical number and not text

1. Annual Chair Report: Process Upon submission deadline, the Dean will review your report. The Dean should schedule a one-on-one meeting with you to discuss your efforts and goals for that year You will both sign the form at time of meeting and both you and the Dean’s Office should keep a copy on file

2. Department Chair Function: Viewing Annual Faculty Reports As Dept Chair, you may view reports that have been submitted from your department by selecting the faculty member by name and the reporting year. You will have the opportunity to provide comments in the text box at the end of the form and submit the comments for the faculty member to view. Upon completion of reviewing reports, you should schedule a one-on-one meeting with each faculty member to discuss their progress and goals for that year and for the next year. At time of meeting, you should print out a hard copy, you and the faculty will both sign at the bottom of the form and keep one copy in your department’s files.

3. Evaluation of Chair Your faculty will be completing an evaluation on your efforts as Chair. All responses will be tallied and reviewed anonymously by the Dean. The Dean will discuss the summary of responses of your evaluation with you.
