5. Scope of Work / Service Requirements 4.2 Scope of Work UTMB desires to solicit manufacturer proposals to establish pricing of all sports medicine products used during surgery. This includes, but is not limited to, instrumentation, arthroscope, implant products, repair instruments (shavers), guidewires, cutters, reamers, fixation devices, screws, punches, ablation wands, tubing, suture, fluid delivery, and other disposable products related to sports injuries and necessary to perform sports medicine procedures (Sports Medicine Products). Price Schedule (Attachment B) Attachment B, Price Schedule is attached to this RFP to give Respondents visibility into previous product purchases for Sports Medicine Products and/or Arthroscopy Supplies. Respondents are asked to perform cross reference work that will allow UTMB to compare their offering of products to those currently being purchased by UTMB. 5

Historically Underutilized Businesses (HUBs) (Section 1.14) UTMB has reviewed this RFP in accordance with the Texas Administrative Code Section (Title 34, Part 1, Chapter 20, Subchapter B, Rule 20.14) and has determined that subcontracting opportunities are not probable under this solicitation. If your company does intend to subcontract any portion of the work, your proposal must include a “HUB Subcontracting Plan” (HSP).
