About Bermuda Bermuda is the most northerly group of coral islands in the world. Bermuda is a British territory, approximately 21 square miles in size. Bermuda has a warm and humid climate, windy especially in the winter months. 70 in April – water about 65. Bermuda is located 580 miles directly east of North Carolina. The main island is covered with low green hills, and surrounded by over 120 small and 6 medium coral islands. Bermuda is the most northerly group of coral islands in the world. Permission form Letters of recommendation (2) Contract Photo release form Phone Chain information form Medical Consent / Authorization form Medical release/ treatment form BIOS Contract BIOS Drug –free zone form BIOS Snorkel Waiver Form Check for $700. Made payable to HBMS. Not required for now but attached as the last page and needed as soon as possible is any prescription med information and the doctors authorization.
