Go to the DEA Website - Read Information pertaining to Application Process for CSOS Certificate –
Form Coordinator
Form Registrant
Form Power of Attorney
All Applications Must Be Notarized.

5. APPLICATION PROCESS
Electronic Certificate Will be Sent to Each Applicant.
Paper Certificate Will Follow via US Mail.
Using Both the Electronic and Paper Certificate- Verification Process Implemented and Electronic Signature assigned.
All Electronic Certificate Holders Need to Install Their Electronic Certificate Signatures on the Designated Ordering PC.

6. SOFTWARE INSTALLATION
Wholesaler Representative Will Provide You With The Necessary Software.
Alert Your IS Department Prior to Installation - They Will Need to Discuss With your Wholesaler Representative All Technical Issues Involved.

7. ORDERING PROCESS
Pharmacy Buyer/Technician Create CSOS Order - No Line Item Limit.
When Ready to Send CSOS Order - Only a Coordinator, Registrant, or Power of Attorney Can Send the Order by Typing in their Unique Password/ Electronic Signature.
