4 Proposed Itinerary Workshops continued: (3) Maintaining “good health” throughout key transitions – health updates and checks Des Chow & Lisa Birtles-Smith Room downstairs / first bar area Masonic Hall (4 )Managing Statements and Plans through transition phases: new ECH plans – the future. Looking at the SEND Reforms agenda Pam Beaumont – Project Officer C Rooms: Main Hall area nearest entrance  (5) Transition for young people with Autism Anne Garcia C Rooms – to the rear part of the main hall  (6) Transition for children with sensory needs Ann Dennison & Maggie Hudson  Room downstairs @ Masonic adjacent to cloakroom

5 Proposed Itinerary 14.00: Back to main hall & coffee 14.10 Main Hall for final presentation: The Early Years Transition Project at The Brow Community Primary School – the Transition Group at work and play & opportunity for questions  14:40 Questions and final messages  Completion of evaluation form  Event end at 15.00
