HHSC has completed the review of CAT 3 feedback forms and selection tools Per our reconciliation, all CAT3 outcome measures in our region have been approved. Providers can move forward with baseline collection plans in preparation for reporting in October 2014.

Original Timeline Sept. 12 th – HHSC provides plan mod & technical change feedback to providers Sept. 26 th – Anchors submit provider responses to HHSC HHSC received almost 2,000 change requests from over 200 providers. Given the volume of requests, the timeline is being revisited with consideration of October reporting

Region 12 had 11 total NMI requests. All metrics were approved. 165 DY2 carry-forward or DY3 metrics approved 24.3 million in funds received in Region 12 for April Reporting

HHSC is working on gathering information on “Why” we need the waiver. We believe they will continue to ask us for Patient Stories Successes thru collaboration CQI System tranformation ◦ Please consider documenting these items

As of July 29, 2014 there are 1,491 approved and active DSRIP projects in the State ◦ 1,274 active 4-year projects ◦ 217 active 3 year projects Automated Reporting system is still being tested. Mid-Point Assessment ◦ Desk Reviews scheduled to take place during September (approx. 300 projects) ◦ Site Visits to certain Providers during Sept & Oct

September 9 th and 10 th – Austin, TX Cindy Mann, CMS Director, will attend the Summit. HHSC will present Snapshot information from all 20 RHPs. Patient Story video from multiple RHPs will be shared at the event.
