2 MAPP is: Mobilizing for Action through Planning and Partnerships A community-wide strategic planning process for improving public health. A method to help communities prioritize public health issues, identify resources for addressing them, and take action.

4 Accreditation Readiness The MAPP process is used in developing Community Health Improvement Plans (CHIPs) CHIPs are one of three documents required to apply for national Public Health Accreditation

5 Fulfill Op Plan Objectives Promote collaboration and transparency by developing region-specific community engagement plans – Seeking community input – Establishing action for Live Well, San Diego!

6 Implementing MAPP Step 1: Establish a planning workgroup to implement MAPP via the Live Well, San Diego! regional forums by November 30, 2011 Step 2: Implement Agency-wide training December 14, 2011 Step 3: Reconvene planning workgroup to review work plan training deliverable (December 2011/January 2012) Step 4: Monitor progress after plan implementation Step 5: Final Product: Live Well, San Diego Community Health Plan
