New Group Home Model for Young Adults with Medical Complexity Spring 2014 St. Mary’s identifies 17 older MFC & MFYA enrolled in SNF in need of complex skilled nursing care. Several hundred more in St. Mary’s home care programs. St. Mary’s partners with Family Residences and Essential Enterprises (FREE) to address the needs of this population. Fall 2014 BIP Transformation Fund Awarded to St. Mary’s in partnership with Family Residences and Essential Enterprises. Awarded to assess and respond to the needs of aging out Medically Fragile Young Adults with I/DD diagnosis.

New York State Balancing Incentive Program (BIP) Transformation Fund was created to provide an unprecedented opportunity for organizations to make strategic investments to transform non-institutional Long Term Services and Supports (LTSS) for individuals with developmental disabilities.

New Group Home Model for Young Adults with Medical Complexity Project Goals Develop a medical supervised IRA model to support MFYA with I/DD who are aging out of pediatric care. Identify initial residents and carry out transitional family planning. Conduct formal assessments of needs of young adults with medical complexity in NYS as well as housing stock currently available to them.

New Group Home Model for Young Adults with Medical Complexity Project Goals Convene a summit to provide an overview of this model and the needs of MFC/MFYA. Identify, secure, and prepare a viable facility to serve as the pilot residence. Recruit and train pilot team members. Open the first medical IRA designed to better serve young adults with I/DD and medically fragile conditions. *Note: some goals conducted with non-BIP funding
