2 Goals of our monthly webinars Our goals for our MFP monthly webinars are:  To provide training on key topics  To better the lines of communication between you and us, the MFP staff  To seek feedback on how to better the processes of MFP/waiver services for our participants  To provide the opportunity to share successes and challenges as RTC’s and MFP transitions

3 MFP Transition Numbers As of today November 12, 2015  186 referrals have been received  59 participants have transitioned to date (15 in 2014 and 44 thus far in 2015) * Our benchmark for 2015 is 47 transitions. * We have met or have exceeded our bench- marks in all waivers except DD and PRTF.

4 MFP Referral Process Submit a referral form Referral form can be accessed on our website @ www.dphhs.mt.gov/mfpwww.dphhs.mt.gov/mfp Once I have a referral form I then call the Mountain-Pacific Quality Health Foundation and request the LOC and Screening Determination and have copy sent to me and the community waiver team. Informed Consent signed and RTC assigned Paperwork for RTC sent out once I receive the signed informed consent. Discussion with RTC about case specifics; participant needs and services; housing options, MFP demonstration services.

5 MFP Companion Services Definition MFP adult and youth participants and/or family members may be paired, when appropriate and available, with a companion to provide support before, during, and following their transitions as well as provide socialization. MFP companions are persons or family members whose life experience provides expertise that professional training can’t replicate. Companions will begin interacting with MFP participants or family members before the transition occurs, will stay in touch regularly, and work to integrate participants into the community and provide support to families. Montana will partner with statewide networks to provide this service. Covered Services MFP Companion Services include assisting a participant with light household tasks (meal preparation, laundry, shopping), supervision, socialization, and community integration.

6 MFP Companion Services (contd.) Limitations: This service is not appropriate for a participant who requires hands-on assistance with personal care needs or hands-on nursing care. State Plan services must be utilized first. This service will not duplicate homemaker services or respite services. Definitions for these 2 services are referenced in the HCBS manual 715/716 (Homemaker/Homemaker Chore) and 730 (Respite Services). The amount of this service approved and provided will vary based on the needs of the individual and has a limit within an individual’s person-centered plan. Companion services cannot be provided by the consumer’s legally responsible relatives or those family members who normally provide the care.

7 MFP Companion Services (contd.) Fees: A participant should identify a companion in his/her plan of care. The companion must be willing to serve as an employee of a provider agency or be an employee of a family who is self-directing their services. MFP Companion Services require prior authorization. Unit of Service: 15 minutes - $4.50 Procedure Code/Modifier: S5136/UA

8 Successful Transition Story  Robie Marcoux. MSW with Lewis & Clark waiver case management in Helena served as the RTC for this participants 2 nd transition. Linda Simmons, SW was the RTC for the 1 st transition.  Participant transitioned from Deer Lodge Health and Rehab – and returned to her apartment in the Helena community in January of 2015. After 6 days in the community she was hospitalized and then returned to Deer Lodge Health and Rehab for the 2 nd time. She transitioned out again to her Helena apartment in July of 2015.

9 Other Items At last … We received our CMS Site Visit Final Report and it is posted on our website  www.dphhs.mt.gov/mfp under the MFP Sustainability tab. www.dphhs.mt.gov/mfp We will be directing you to utilizing the expertise of your RPO’s in making the determination for DME for MFP participants. On November 4 th I presented MFP to the NH Administrators, SW and discharge planners on their quarterly webex and discussed the eligibility requirements and the MFP referral process. Brian will be presenting housing options to this same group on Wednesday December 2 nd. On our December 10 th webinar we will be introducing the MFP Demonstration Services Cost Sheet.
