* Making 112 referrals to a medical home, providing over 200 hours of donated care, assisting patients with overcoming barriers to obtaining services through over 500 transportation vouchers, and providing 38 vaccinations - Johnson County Public Health. * Improving health outcomes for a patient that had presented to the Siouxland Community Health Center walk-in clinic after visiting the emergency department for symptoms of diabetes. Within three months of becoming a patient at Siouxland Community Health Center, the patient’s HbA1C level was below 6% and no longer required insulin to control the patient’s diabetes. * Using conservative figures, for every $1 used to administer the VPN, $24.66 of free specialty care services were donated to patients.
