One short bus ride away/provide transport Familiar/approachable setting Non-medical settings/community settings Choice of centres is important What is included a) Nutrition Multi-cultural advice about food (culturally relevant) Vouchers for healthy foods at local supermarkets Sessions on portion sizes, food swaps, food combining, what to eat at what times of day Free cooking classes (for all family!) Salad bar on site Food articles- up to date research in lay language Interesting, diverse recipes Call people using this service - service users or clients not patients Can a friend or family member access some elements to support client? Service to operate as a rolling programme rather than having to wait for start of next service

2 Referring to the serviceWhat is included b) Exercise Free gym and varied exercise sessions dedicated to people in the service Exercise classes to be culturally relevant – e.g. different dance classes One to one classes and help at gym for people who need/ want it Group yoga and mindfulness sessions Taster sessions so people can see what they prefer Pilates and other exercises to manage and reduce pain and strengthen core Organise group sessions for people at same fitness level (e.g mobility problems or LTCs) Pedometer for daily use to assess activity level Personalised exercise plans Outdoor competitive sports and other fun activities What is included c) ‘Talking sessions’ Must be a highlighted element of the service Should be mixture of 1:1 and group support sessions (Start one to one then progress to group sessions) Include a ‘community’ of people who have used the service to encourage sustainable support afterwards Addiction services implemented into therapies Needs to be longer than 12 weeks for some people Men and women only talking groups (separate) A range of different talking sessions available e.g. hypnosis, counselling, CBT What other services are linked in? Assistance with hygiene Physically active volunteer/ work opportunities (e.g. training children, football coaching) Tackle other issues such as debt, benefit and housing Sign-posting and referral to pain management services Access to saunas, swimming and massage therapy Evening and weekend classes available Hydrotherapy pool Relapse triggers and plan

3 Referring to the serviceSustaining and maintaining Outcomes Use of text messages and apps to keep motivation once started using Day trips together as a group- walking, food shopping and socialising Annual reviews Keep in contact with obesity nurse, GP and support group after service has finished Award/ recognition/ certificate at milestones and after completing the service Champions/ activists programme to promote service within communities for clients who complete Lose weight Maintain weight loss Health conditions improve reduced medication More energy and > levels of fitness Feels healthier and happier Improved mental health Feel more in control More mobile and physically active Confidence boost and better self esteem More educated regarding nutrition and exercise Able to educate other people including family The right amount of support to now go on and do it by themselves Changed eating habits Know how to access information/services that will continue to help Ready for employment Improved love life Given children a better start in life “To live again”
