2 Purpose: Healthy Data Habits  Ensure data is inputted and extracted from EHR accurately  Accurately reflect that care is delivered  Identify gaps and opportunities

3 What to Do? 1. Identify and document data uses 2. Establish data quality benchmarks 3. Create Data Quality Improvement Plan 4. Measure progress & adjust goals 5. Maintain formal checks on data quality 6. Develop Sustainability Plan

4 1. Identify and Document Data Uses a. Build working relationship with practice team  Who is your point of contact? b. Data Uses – Identify and document  How are practices already using data to measure patient outcomes? c. Systems and Inputs –  Which data is internal vs. external? (i.e. vitals v. labs) d. Impact –  What is the overall effect on the practice? e. Prioritize –  What are the important/easiest gaps to fill?

5 2. Establish Data Benchmarks a. Determine data elements to target  Where are the data points coming from? b. Retrieve relevant data  How will you get the data out? What other impacts c. Compare data  Where are the deltas? d. Determine quality issues  How to validate the data? What issues may arise? e. Establish base line of data  Did you coordinate with the practice’s PF?

7 3. Create Data Quality Improvement Plan NOTE: Completion of data quality improvement plan is trigger for CHITA payment a. Identify goals for data quality improvement  Map to ENSW data priorities b. Institute team check-ins for tracking feedback  Over communicate  Loop in practice facilitator c. Be sure to include QI team d. Establish measures of success

8 4. Measure Progress & Adjust Goals a. Compare progress to benchmarks b. Alter plans as needed c. Celebrate successes!

9 5. Formal Data Quality Checks  Review initial plan  What can help monitor and improve data quality? Processes Technology
