7 Commonwealth Care Alliance Nonprofit, Medicare Advantage special needs clinical program Focus on medical AND social support needs Members are frail and medically complex Dually eligible for Medicare and Medicaid Live in communities with significant disparities in healthcare services 85% of Western MA members certified to be in Nursing Homes but live at home with support of clinical teams
