Control 2: Guards and protection devices shall be implemented to avoid unwanted collision or falling down of operators Control 3: Inside the Base and the Cabin an adequate level of light shall be provided. Control 4: While working on lifting trolley the operators shall wear safety belt to avoid dangerous falling down. Control 5: Labels and indications must highlight safety concerns and requirements: Use of safety devices such as protective helmets, safety shoes, safety belts, etc Operational requirements “Mind your head” or “mind your step” signals. The labels must be both in English and in Spanish Control 6: The platform, the stairs and the floors of Base and Cabin shall be designed and constructed to prevent persons slipping. 3.METHOD FOR VERIFICATION OF CONTROL(S): Verification of Control 1: Review of Antenna instruction manual

Control 2: Guards and protection devices shall be implemented to avoid unwanted collision or falling down of operators Control 3: When the Antenna is moving at high speed no personnel shall be on it or in the vicinity (3 m from the base). Control 4: While working on Antenna for maintenance, the Antenna shall be stopped in a safe condition. Control 5: Labels and indications must highlight safety concerns and requirements: Use of safety devices such as protective helmets, safety shoes, safety belts, etc Operational requirements “Mind your head” or “mind your step” signals. 10 Reproduction interdite © ALMA EUROPEAN CONSORTIUM Reproduction forbidden Design, Manufacture, Transport and Integration in Chile of ALMA Antennas Page 12 ALMA-01 Cause 3 1.HAZARD CAUSE DESCRIPTION: Collision of operator or damage to Antenna during integration or transportation (hoisting/handling devices rupture/operators error) SEVERITY: ILIKELIHOOD: D 2.CONTROL(S): Control 1: Where the weight, size or shape of Antenna various component parts prevents them from being moved by hand, the part must; - either be fitted with attachments for lifting gear, or - be designed so that it can be fitted with such attachments (e.g. threaded holes), or - be shaped in such a way that standard lifting gear can easily be attached. Control 2: Where machinery or one of its component parts is to be moved by hand, it must: - either be easily movable, or - be equipped for picking up (e.g. hand- grips, etc.) and moving in complete safety. Control 3: The lifting accessories must be designed and constructed to withstand the overload in the static tests without permanent deformation or patent defect. The calculation must take account of the values of the static test coefficient chosen to guarantee an adequate level of safety: that coefficient has, as a general rule, the value of 1.5. Control 4: All lifting accessories must show the following particulars: — identification of the manufacturer, — identification of the material (e.g. international classification) where this information is needed for dimensional compatibility, — identification of the maximum working load, In the case of accessories including components such as cables or ropes, on which marking is physically impossible, the particulars referred to in the first paragraph must be displayed on a plate or by some other means and securely affixed to the accessory. The particulars must be legible and located in a place where they are not liable to disappear as a result of machining, wear, etc., or jeopardise the strength of the accessory.

Ropes must not comprise any splices or loops other than at their ends; Where chains with welded links are used, they must be of the short-link type. The working coefficient of chains of any type is chosen so as to guarantee an adequate level of safety; this coefficient is, as a general rule, equal to four; The working coefficient for textile ropes or slings is dependent on the material, method of manufacture, dimensions and use. This coefficient is chosen so as to guarantee an adequate level of safety; it is, as a general rule, equal to seven, provided the materials used are shown to be of very good quality and the method of manufacture is appropriate to the intended use. Should this not be the case, the coefficient is, as a general rule, set at a higher level in order to secure an equivalent level of safety. Textile ropes and slings must not include any knots, connections or splicing other than at the ends of the sling, except in the case of an endless sling; All metallic components making up, or used with, a sling must have a working coefficient chosen so as to guarantee an adequate level of safety; this coefficient is, as a general rule, equal to four; In order to verify that an adequate working coefficient has been attained, the manufacturer or his authorised representative must, for each type of chain, rope or component used directly for lifting the load, and for the rope ends, perform the appropriate tests or have such tests performed. Control 6: Transport, lifting, hoisting devices and similar equipment shall be approved by an officially recognized independent verification agency (TÜV, UL, Bureau Veritas or similar institution).
