3 Incident Reporting Definitions: Incident - An event that occurs which has the potential to cause serious personal injury/ danger to health or damage to property. Accident - An event that occurs and results in personal injury or illness requiring medical attention. Dangerous Occurrence/Near Miss - An event that has the potential to cause personal injury or damage to property or the environment.

4 Definitions Work injury means:  an injury to a person that requires first aid or medical treatment if the injury was caused by work, a workplace, a workplace activity or specified high risk plant  the recurrence, aggravation, acceleration, exacerbation or deterioration of any existing injury in a person if: - first aid or medical treatment is required for the injury - work, a workplace, workplace activity or specified high risk plant caused the recurrence, aggravation, acceleration, exacerbation or deterioration  any serious bodily injury, if the injury was caused by work, a workplace, a workplace activity or specified high risk plant.

5 Definitions Serious bodily injury means an injury to a person that causes:  the injured person's death  the loss of a distinct part or an organ of the injured person's body  the injured person to be absent from the person's voluntary or paid  employment for more than four normal working days.

6 Definitions Work caused illness means:  an illness contracted by a person to which work, a workplace, a workplace activity or specified high risk plant was a significant contributing factor  the recurrence, aggravation, acceleration, exacerbation or deterioration in a person of an existing illness if work, a workplace, a workplace activity or specified high risk plant was a significant contributing factor to the recurrence, aggravation, acceleration, exacerbation or deterioration.

7 Definitions Dangerous event means:  an event caused by specified high risk plant  an event at a workplace caused by workplace activity

8 Definitions If the event involves or could have involved exposure of persons to risk to their health and safety because of:  collapse, overturning, failure or malfunction of, or damage to, an  item of specified high risk plant  collapse, or failure of an excavation or of any shoring supporting  an excavation  collapse, or partial collapse of any part of a building or other structure  damage to any load bearing member of, or the failure of any  brake, steering device or other control device of, a crane, hoist, conveyor, lift or escalator  implosion, explosion or fire escape, spillage or leakage of any hazardous material or dangerous goods  fall or release from a height of any plant, substance or object  damage to a boiler, pressure vessel or refrigeration plant  uncontrolled explosion, fire or escape of gas or steam.

12 Immediate Action  If Medical Treatment is required, First Aider to attend to Injured Employee or arrangements are to be made for Employee to attend review with medical practitioner.  For Serious Personal Injury, Supervisor/ First Aider will call 000 for Emergency services and employee must be accompanied to Hospital.

13 When  Incident is reported to Supervisor as soon as possible after event.  Incident report form is completed and given to your Supervisor, include all possible witness statement, photos etc.  Supervisor, Leading Hand/Team Leader, WHSR complete incident investigation reporting their findings to management. (always ensure feedback from people at worksite is included).

14 Notifiable Incident A notifiable incident is an incident resulting in:  a person suffering a work injury that is a serious bodily injury, including a fatality  a work caused illness  a dangerous event.

15 When A notifiable incident must be reported:  within 24 hours of being made aware of the incident  in writing on the approved form.  If the incident involves a fatality immediate notification is required.  This can be done by phone or fax.  Ring 1300 369915

16 Who Who is responsible for reporting an incident The following people have a legal obligation to report particular workplace incidents not causing death:  employers  self employed persons  principal contractors (at a construction workplace)  electricity distribution entity.

17 Who Incidents are to be reported to your supervisor Immediately  Completed forms are to be submitted to WHSQ within 24 hours  Supervisor (deaths to be reported by phone within 2hrs) OHS Committee review all incidents  All incidents must be reported to: - Supervisor - WHSR - AFHS Management

18 Why Why do we investigate an incident?  To identify what happened, how it happened and why it happened.  To identify control measures to prevent a recurrence  Reduces injury rates  Identifies deficiencies in the current system  The Incident Investigation is not used to place blame

19 Why What to we Investigate?  Incidents  Near Misses  Dangerous Occurrences Who Investigates?  Employee  Safety Representative  OHS Coordinator  Department Manager  Supervisor

20 How  Plan the investigation  Identify who must be involved  Conduct the investigation at the incident scene within 24 hours of the incident occurring  Document the outcome on the report form. Photo’s should be obtained  Collect statements from witnesses

21 How Determine those Factors Essential to the Incident  Unsafe plant/equipment  Staff not following procedures or adopting hazardous work practices?  Poor housekeeping, inadequate lighting? Contributing Factors   Did the person not follow procedures?   How did the hazardous conditions occur?

22 How When determining corrective actions consider:  What will be most effective  What will be the most cost efficient  What is the quickest / easiest to implement Corrective actions should be monitored to ensure that:  The desired outcome has been achieved  The corrective actions have not created new problems

23 Summary Incident Report  Ensure all items are completed  Pay particular attention to:  Incident Type  Incident Causes  Corrective Action  The detailed description of Accident
