AFS 2002:1 Risk analysis Risk analysis shall be performed before taking into operation - finding all hazards originated from the complete installation - definining the scope of the continous surveillance

AFS 2002:1 Risk analysis Could be performed by: Own staff which is separated from the production Own staff which in another way may perform an objective assessment (preventive maintenance) Experienced consultant (own competence should be pursued)

AFS 2002:1 Renewed risk analysis Shall be performed when: - the plant (installation) - the pressure equipment - general conditions have been changed in a way that new hazards may have occured or existing hazards have been affected

AFS 2002:1 Continous surveillance -definition ”Regular supervision of pressure equipment and safety equipment in order to be sure that the safety is adequate” ”The goal for continous surveillance is to be reasonably assured that equipment which are inportant for safety maintain its performance and works safely”

AFS 2002:1 Updating of program for continous surveillance Shall be updated at least every year If deficiencies are found, which have not been taken care of, the plant must not be used until actions have been taken. Assessment must also be performed in order to judge if routines for follow up need to be changed

A specially appointed person shall be responsible for the work of continous surveillance. This appointed person shall have: - understanding of the process - understanding of the pressure equipment in the plant - understanding of how the continous surveillance shall be performed AFS 2002:1 Personnel for continous surveillance

AFS 2002:1 Continous surveillance of gas cylinders Gas cylinders shall be continously looked after in order to be sure that the safety is adequate It shall include: - visual outside inspection of the gas cylinder - visual inspection of the fittings If damage is found, the gas cylinder shall immediately be shut off

AFS 2002:1 Repair or change Routines shall be present Written permission shall always be obtained when performing work inside a pressure equipment If the plant or any of the equipment will change during repair or change, the program for continous surveillance shall be adjusted

AFS 2002:1 Follow up accidents and incidents Accidents and incidents shall be documented and investigated. Necessary action resulting from the investigation shall be taken. Operation of the plant must not be resumed until it is established that the plant is safe

AFS 2002:1 Marking of equipment Piping and other pressure equipment shall be marked with name of fluid All piping shall also be marked with flow direction Safety equipment shall be marked regarding function Main shut-off valves, emergency stop and its actuators shall be marked in a way which ensures that appropriate measures can be taken quickly in an emergency Actuators must be provided with information giving clear indications on how the equipment is operated
