Beneficiary feedback System Overview District/CO Team Leader & Emergency Response Manager Role: Receive status report on all feedback every 2 wks and follow up with Program manager if necessary District/Sub-district Field Officer - Health/WASH/Food/livelihoods/Ed Role: 1. Collect feedback, and pass on to POC 2. Receive request from PO and take action 3. Inform beneficiary of resolution and informs PO of status District/Sub-district Program Officer – EHN/WASH/Food/livelihoods/Education Roles: 1. Receives feedback request from POC 2. Assigns request to FO for action 3. Receives resolution and status from FO and informs POC of resolution and status PM Program Manager - EHN/WASH/Food/livelihoods/Educatio n Roles: 1. Copied on feedback request POC enters into database 2. Follows up with PO if necessary if resolution late IT Assistant (System point of contact - POC) Role: 1. Enter feedback request from FO into database 2. Route feedback to PO 3. Update resolution and status in database 4.
