How does it work? Assess patient’s tobacco status and interest in quitting Complete referral form and fax to Central Triage Central Triage identifies appropriate Quitline Quitline contacts patient Patient outreach results returned to you

Patient Experience Individually tailored calls from a quitline coach Unlimited calls to a quitline coach Program materials via mail Prescription consultation (nicotine replacement therapies) Over-the-counter (OTC) fulfillment* *costs vary depending on the individual’s benefit coverage

Provider Benefits It’s a free program for both clinics and patients. Make patients referrals with a single fax. Monitor your patient enrollments.

A Brief Tobacco Intervention: The 5As Ask about tobacco use. Advise the patient to quit. Assess readiness to quit. Assist the patient in quitting. Arrange for follow up.

How can I get started? 1.Register your practice at www.health.mn.gov/callitquits.www.health.mn.gov/callitquits 2.Once confirmed, you can refer patients immediately. 3.Additional resources well be mailed, but are also online. 4.Monitor your referrals, as needed. 5.We’ll send quarterly updates with additional tips and tools.
