Lessons Learned/Key Insights Test forms and processes in one area first to iron out the wrinkles and get staff buy-in. Test one change at a time Stay positive – results don’t happen overnight Evaluate each change implemented in order to achieve continuous quality improvement sustainability U sing the multidisciplinary approach provides valuable insight into all preventative strategies The resident safety audits has ensured staff adhere to Best Practice Guidelines in Falls Prevention

Next Steps Key Sustainability Steps/Plan:Target Dates Continue to identified residents at high risk for falls On going Initiate and continue with admission RN to administer FRAT with all new admissions March 07, 2011 Continue application of falling leaf logo above bed, name plate & mobility devices On going Initiate and continue with administering FRAT by RN on change of health status, readmission from hospital April 30, 2011 Implement on unit post fall huddles with all disciplines at time of fall March 15, 2011 Continue to trial alternative fall prevention strategies in order to meet the specific residents risks (e.g. motion lighting, sensors, non slip flooring, Hi- Lo bed, medication reviews) On going
