4  Materials Diary entry report.  Now can also be used to reference an external document like a certification.  This will eliminate need for using the “900” report.  Features Bid Item “Pull Down” menu.  Can reference multiple documents per entry.  Will auto fill Doc. I.D. with bid item or can enter own I.D.  Write Doc. I.D. on document and insert in binder behind e-guide sheet. 4

6 Certification of Compliance: A Certification of Compliance shall be furnished (for each contract) by the guardrail installer for each manufacturer of beam guard for the contract. The certification needs to be have the date, signature and title of the person certifying the product. Information on the certificate shall include the following: State Project ID Highway Number Project Description County Name or Brand of Manufacturer Product Identification Number (ie: AASHTO M180 Class A Type II) Signed Buy America Certification Statement* *Note about Buy America Certification: Certification statement needs to state where the steel/iron has been melted, molded, galvanized, epoxy coated etc. A signature is required on the Buy America certification. Also, mill and manufacturer certification is expected to be on file and provided upon request. Buy America is a mandate of the Federal Highway Administration, enacted in 1983. All guardrail materials and associated guardrail components are to meet the material requirements of WisDOT Standard Specification, Section 614 and AASHTO Specification M180. Any guardrail material not in conformance with specifications will be removed and replaced at no cost to the Wisconsin Department of Transportation. The engineer will verify that the Certification of Compliance Product Identification number is on the shipment of beam guard delivered to the project site. 6

10 10 614.2.5 Posts and Offset Blocks 614.2.5.1 Wood Posts and Offset Blocks (1) Furnish sawed posts and offset blocks of one of the following species: Douglas fir Southern pine Ponderosa pine Jack pine White pine Red pine Western hemlock Western larch Hem-fir Oak (2) Ensure that posts are the size the plans show and conform to the nominal and minimum dimensions tabulated in 507.2.2.3. The contractor does not have to surface the posts. Provide posts of the net length the plans show after setting and cut off. (3) Use stress graded posts rated at 1200 psi fb or higher. Determine the stress grade rating for douglas fir, western larch, and southern pine as specified in 507.2.2.4. C & M 8-50 Requirements POSTS, TREATED WOOD Guardrail Right of Way (Security Fence) Screen Fence Sign, Etc. Field A manufacturer's certification of treatment must be made available by the contractor upon request of the engineer. A diary entry is required for these items. ** Stay tuned, I would recommend requesting certification for Beam Guard Posts to validate conformance to these requirements.

11 715.2 Materials 715.2.1 General (4) Ensure that the concrete mix report includes a cover sheet with signature blocks for both the mix developer and the engineer. Have the mix developer sign and date each copy attesting that all information in the report is accurate. The engineer will sign and date each copy of the report. The engineer’s signature verifies that the engineer had the opportunity to review the mix report, to check that it meets the concrete mix requirements, and to comment. The engineer will return a signed copy to the contractor within 3 business days of receiving the report. C & M 8.35.1.2 Requirements: 8-35.1.2 Contractor Concrete Mix Design The contractor is responsible for providing the design of the concrete mixture for use on the project and for any necessary adjustments during production. A mix design may be a new design or one used on a previous project. New mixtures are those that use different material sources or quantities than a previously used mix. A PCC Technician II, hired or employed by the contractor, is required to develop and submit the mix design report to the engineer before the production of concrete for the project. The mix design must meet the conditions specified in the QMP provision. The report can include a number of different mix designs, but each mix design is required to have supporting laboratory or field test results. Multiple mix designs will enable the use of the most appropriate mix on a project, for given conditions. The contractor must complete department worksheet WS5014, Concrete Mix Design, and submit to the engineer. The engineer’s signature verifies that the engineer had an opportunity to review the mix design. New WS form will be included with next C & M update, please use “new” form and have contractor fill out completely. New form example along with updated “Ancillary Concrete Daily Test Report”. 11

14 14 APRIL 2013 BUY AMERICA PROVISION All steel and iron materials permanently incorporated in this project shall be domestic products and all manufacturing and coating processes for these materials from smelting forward in the manufacturing process must have occurred within the United States. Coating includes epoxy coating, galvanizing, painting and any other coating that protects or enhances the value of a material subject to the requirements of Buy America. The exemption of this requirement is the minimal use of foreign materials if the total cost of such material permanently incorporated in the product does not exceed one-tenth of one percent (1/10 of 1%) of the total contract cost or $2,500.00, whichever is greater. For purposes of this paragraph, the cost is that shown to be the value of the subject products as they are delivered to the project. The contractor shall take actions and provide documentation conforming to CMM 2-28.4 to ensure compliance with this “Buy America” provision. http://roadwaystandards.dot.wi.gov/standards/cmm/cm-02-28.pdf#cm2-28.4 Upon completion of the project certify to the engineer, in writing using department form WS4567, that all steel, iron, and coating processes for steel or iron incorporated into the contract work conform to these "Buy America" provisions. 16 WisDOT staff can get assigned testing equipment through the Region Labs. This can include shovels, air meters, slump cones, thermometers, Infrared thermometers (heat guns) etc. Rhinelander office see Dean Gritzmacher or John Brophy…Rhinelander Lab phone 715-365-5735 Wisconsin Rapids office see Pat Shuda or Jeff Michalski…Rapids Lab phone 715- 421-8060 All field staff can get sampling supplies from each Region Lab, such as Sample Bags, Sample Cards, AC cans, Tack Jugs, Cylinder Molds and Caps. All field staff can utilize each Lab to test samples. Samples can be dropped off during normal hours or can be placed in the Lab Entry in Rhinelander or Drop Off Room in Wisconsin Rapids. Door combinations can be obtained through lab staff or your P.M. Field Samples intended for Madison can be dropped off at each lab along with completed sample cards and they will get shipped out from there. Nuclear Gauges and Validators can be signed out through each Region Lab. Check with each lab for specific sign-out and transport needs. HMA mix samples will be tested at each Region Lab. Coordinate paving schedules with Dean and Pat as soon as dates and times are known. 16
