5. COMMUNITY HEALTH PARTNERSHIPS Primary care in Scotland is organised into Community Health Partnerships. Health professionals work together to improve the lives of people in their area. There are ten CHP’s across the Greater Glasgow area. Each CHP provides primary care services such as: health visiting district nursing speech and language therapy mental health nutrition services
