3 Mission Statement 3 The Mission of FLPN Inc. is to support and empower families so that they can improve the quality of their lives and help their children to achieve their full potential within the community.

4 4 We, the Finger Lakes Parent Network, Inc. envision an extended community of families empowered to interact with various community agencies in a manner that cultivates solutions and eliminates barriers to service for children with emotional, behavioral, mental/or developmental disorders. The extended community is an environment where new voices are valued and the focus is on family strengths and needs, and where current information and adequate resources are available to support and empower families.

9 Self-help support groups with trained peer facilitators and on- site childcare are in all nine counties. Groups meet based on individual county schedules. O ne group is a caring/sharing and the other is educational. Group activities are planned with input from families. Each group offers recreational childcare during support meetings to identified children and siblings. Family Support Groups with Childcare 9

10 Advocacy Family Peer Advocates provide advocacy to families at school and agency meetings. 10

16 Home and Community Based Waiver Services 16 What is Waiver? Medicaid funded Home and Community-based Services are individualized, culturally competent, family driven, and community based. The target population is children at risk of out of home placement who have complex and significant needs. To qualify, the family income is “waived” to allow children to receive needed services. We contact families at least twice by phone, once by letter to encourage engaging in our services. Referral source will be notified within 10 business days regarding status of the referral.
