Alternatively, e-mail Sam Sanchez (ssanchez@ptc.com) or Jan Bauer (jbauer@ptc.com)ssanchez@ptc.comjbauer@ptc.com l Include all student info: First Name, Last Name, E-mail Address (IMPORTANT!), Fax, Phone,Company Name, and Company Address  Report no-shows using standard attendance function  Mark remaining days’ attendance: Each day as it comes All at once (on last day of class)

Learner accesses evaluation through “My Evaluations” link on main page. 3. Learner must first accept or decline the evaluation. 4. After accepting, learner may elect to complete the evaluation anonymously. 5. Preferably, have learner complete the evaluation on last day of class. Otherwise, have him/her complete it when he/she regains internet access.
