5. 5 Shipping Labeling Violation (1) Contrary to the regulations, the licensee made a shipment of radioactive material without properly affixing the markings to the package. During transit the markings and labeling apparently came off. As a result, the cask was being transported without being properly marked and labeled.

6. 6 Shipping Labeling Violation (2) The licensee made seven shipments of radioactive material without including the required information on the shipping papers. Multiple and various deficiencies in the labeling were identified in these shipments.
