2 Major Program Changes in 2007 Improvements to Inspection Report issuance timeliness Completion of all inspection procedures at each facility each cycle (2 inspections per cycle) Inspections now conducted within the primary interval (12 or 24 months) without reliance on 25 percent grace period

3 Performance Measures Inspection reports issued within 30 days = 98.3% Inspection reports issued within 45 days = 100% Class 1 Inspection cycles completed within 12 months = 100% Class 2 Inspection cycles completed within 24 months = 100%

5 Shipping Labeling Violation (1) Contrary to the regulations, the licensee made a shipment of radioactive material without properly affixing the markings to the package. During transit the markings and labeling apparently came off. As a result, the cask was being transported without being properly marked and labeled.

6 Shipping Labeling Violation (2) The licensee made seven shipments of radioactive material without including the required information on the shipping papers. Multiple and various deficiencies in the labeling were identified in these shipments.

7 Operator Not in Requal Program One licensed senior reactor operator did not take requalification written examinations or annual operating tests between 1998 and 2004

8 Inadequate Experiment Approval Four experimental samples were irradiated in the reactor pneumatic irradiation experimental facility that were not reviewed and approved in accordance with written operating procedures.

9 Emergency Plan Staffing Emergency Plan staffing commitments not met Two of the three positions designated in the plan to be Emergency Director were no longer staffed and the third designee was not readily available to perform duties had there been an emergency

10 Operator Control Operations NCVs Two Non-Cited Violations (NCVs) for failure to have an operator at the controls as required. One NCV for failure to maintain the console key properly locked in the key box
