How we got here Background of Austin/Travis County CHA and CHIP MAPP Model 3

Community Health Improvement Planning Process August 2011- December 2012 National Association of City and County Health Officials (NACCHO) awards grant to build community support for a CHA/CHIP and required a robust CHA be developed. January – June 2012 Community Health Assessment (CHA) is conducted to identify the health related needs and strengths of Austin/Travis County. July – December 2012 Community Health Improvement Plan (CHIP) is drafted. CHIP is a long-term, systematic effort to address public health problems and is based on CHA results. January – June 2013 Developed Implementation for CHIP (I-CHIP) Year 1 Action Plan, a clear sequence of steps to achieve strategies. July 2013 – Present Implementation of the CHIP and public forums or presentations. 4

Initial update on CHA/CHIP development At the CIC Conference in 2012, on behalf of A/TC Community Health Improvement Planning, Lawrence Lyman with Travis County Health and Human Services and Veterans services, presented the CHA/CHIP work. 7

CHIP’s Structure Lasting community change stems from critical assessment of current conditions, an aspirational framing of the desired future, and a clear evaluation of whether efforts are making a difference. The four priority areas in the CHIP are outlined with the following information: Goals Objectives Strategies Potential Outcomes and Indicators both long and short term 11

Lessons Learned Diversity in how priority areas are addressed and flexibility within the CHIP to accommodate these differences. Ongoing identification of key partners. Developing structure allows partners to better contribute. Partner ownership of CHIP implementation is critical. Evaluation of all aspect throughout the entire process. 13

Strategic organization structure Departmental re-organization to focus on: Community need Planning Evaluation Alignment of department’s strategic planning and CHA/CHIP planning Restructure and planning for CHA/CHIP influenced by public health accreditation measures. 17

Next Steps January 2016, mid-CHA and CHNA will be jointly planned and conducted by local non-profit hospitals, healthcare district and local health department June 2016, last month of the first iteration of Austin/Travis County Community Health Planning (CHA/CHIP) July-December 2016, evaluation of the first iteration with recommendations for improvement January 2017 next CHA begins 18
