Today’s Presentation This is the one time of year when you can update and change your benefits plans without a “life-event”. Today we will guide you through the enrollment process for Medical Insurance and Dental Insurance.

What is Open Enrollment This is the one time of year when you can update and change your benefits plans without a “life-event”. “Open” means any Eligible Employee may begin participation regardless of past coverage. These plans include: Medical Plan Dental Plan

Get The Most Out of Your Benefits This is a good time to: Check your coverage amounts Add or reduce coverage Review and if Necessary change your beneficiaries Life and Accident Insurances – Whoever you choose Add or remove covered family members

Dental Insurance Coverage Your Dental insurance coverage will be will be supplied by; Starting January 1, 2012

Your Cost Employees will be responsible for the following amounts deducted from each paycheck based on the level you choose; Low Option Dental Coverage; Employee Only - $18.50 Per Pay Period Employee and Spouse - $28.35 Per Pay Period Employee and Children - $38.30 Per Pay Period Employee and Family -$73.20 Per Pay Period High Option Dental Coverage; Employee Only - $25.62 Per Pay Period Employee and Spouse - $34.28 Per Pay Period Employee and Children - $44.50 Per Pay Period Employee and Family - $94.64 Per Pay Period

What to Turn In In order to ensure your Dental Insurance begins on January 1, 2012 you must have the following forms; o Form #1002 – Dental Insurance Election o Form #1005 – Dental Insurance Dependent Information o Form #1022 – Payroll Authorization Form - Dental Forms must be turned in to Human Resources by November 22, 2011!

Medical Insurance Coverage Your Medical insurance coverage will be will be supplied by; Starting January 1, 2012

The Good News! The Board of Directors has elected to postpone second phase of increase for 2012. Board recognized hard work employees did in trying to control claim costs. Cost for coverage will not increase this year. The employee’s share will be the same as last year. Those with Employee and Spouse Coverage will pay $20.20 per month and those employees with Employee & Family Coverage will pay $95.90 per month.
