3 Teacher Evaluation Process
Required Forms: Self-Reflection: Due September 30th Beginning of Year Conference: Due October 15th Pre-Conference Form Formal Observation Summative Evaluation Non-Tenured: Two evaluations due 12/10 and 5/1 Tenured: One evaluation due 2/15

. Completed by all tenured and non-tenured teachers Aligned with Rubric Required Components in Grey Due September 30th

5 Beginning of the Year Conference
Principal and teacher meet to review Self-Reflection Identify artifacts to be provided by teacher (1 minimum for each of the 10 required components) Artifacts Profile facets of teaching students, delivery of service, or areas of professional practice Artifacts may be used as evidence to establish a rating in a specific domain and component Observable Evidence by Supervisor At discretion of principal or by mutual agreement additional components may be added Due October 15th
