2 Our goals for our MFP monthly webinars are: To provide training on key topics To better the lines of communication between you and us, the MFP staff To seek feedback on how to better the processes of MFP/waiver services for our participants To provide the opportunity to share successes and challenges as RTC’s and MFP transitions Goals of our monthly webinars 2

3 MFP Transition Numbers As of December 10, 2015 201 referrals have been received 63 participants have transitioned to date (15 in 2014 and 48 thus far in 2015) * The transition benchmark for 2015 is 47 transitions. * MFP met our 2015 benchmarks of 47 on Friday, December 4 th. Since then there has been 1 additional transition that occurred and 2 other are scheduled before the 18 th of December. With 2 weeks left in the year!. 3

9 Definition: Montana is a rural/frontier state, with many Montanans living far from services, neighbors, and family. Montana will be using MFP as an opportunity to increase its use of technology to connect participants to health services, families, peer and social opportunities. If required by the person-centered plan:  Montana may support communication needs including hardware and associated costs to maintain connectivity or provide enhanced supervision, monitoring and diagnostic needs.  Montana may purchase information technology equipment or software needed to support medication management.  Montana will seek out group purchasing for these products and services for participants. Covered Services: MFP Connecting Information Technology Services includes: Purchase of technology devices (ex. cell phone, smart phone, computer, i-pad or tablet) Remote monitoring equipment; Purchase of software; set up fees; monthly fees Purchase of medication dispensing devices 9 MFP Information Technology

10 Covered Services : (continued) Remote monitoring equipment is equipment used to operate systems such as live feed video, motion sensing system, radio frequency identification, web-based monitoring system, alarm systems, or other approved devices. It also refers to the equipment used to engage in live two-way communication with the person being monitored. Limitations: The payment is not supported by MFP funding beyond participation in the MFP program. The service is not reimbursable under State Plan Durable Medical Equipment This service will not duplicate existing waiver services. Service Requirements: The use of technology devices is intended to connect participants to health services, families, peers, and social opportunities. It must be included in the person centered plan. 10 MFP Information Technology

11 Fees: Prior authorization is required for this service. This is a cost-based item is built into the annual cost plans. It will be paid at actual cost within the payment parameters outlined below. Information Technology Purchase (includes software) Unit of service: 1 Rate: $800 (or actual cost up to $800) Connecting Information Technology Set Up Unit of Service: 1 Rate: $100 Connecting Information Technology Monthly Unit of Service: 1 – capped at 12 units Rate: $58 (x 12 = $696) Monitoring, Diagnostic Information Technology Unit of Service: 1 Rate: $700 Monitoring, Diagnostic Information Technology Monthly Unit of Service: 1 – capped at 12 Rate: $150 (x 12 = $1800) 11 MFP Information Technology

14 Successful Transition Story Sue Neff, SW with Spectrum –waiver case management in Butte served as the RTC. Participant transitioned from Deer Lodge Health and Rehab –to an apartment in the Butte community in June of 2015. 14

15 Other Items On the HCBS participant’s service plan (ED, PD, SDMI) please make sure that the MFP Demonstration services that were utilized for the community transition are listed. Next webinar will be on Thursday January 14 th at 1:30 p.m. 15 Happy Holidays!
