Incidents and Analysis CountryClientDateClassification Tajikistan (Crew 0141) BOCJan. 9, 2015RWC Brief description of incident At about 12:30 on Jan. 9, 2015, after tamping a hole, one loader helper carried two poles on his right shoulder and walked down the slope to the next shot point by himself without waiting for his two workmates. Since the next point was at the bottom of the hill, the loader helper selected a shortcut to walk straight down the slope instead of the zigzag route. After walking 4-5m down, his left foot was tumbled and made a roll on the slope. Then the IP stood up by himself without feeling painful and continued to work for the next point. After lunch, the IP felt painful and the left collarbone area was swollen. Then he was sent to the local hospital for X-ray examination and treatment. After examination, it was found that the IP got hairline fracture on his left collarbone. Restricted work day: 30 days.

8 2. Incidents and Analysis The IP selected shortcut and descended straight down the slope. The IP was tumbled and slipped down from high to low side.

9 2. Incidents and Analysis Causes: The IP select a shortcut on a steep slope instead of the zigzag route. The IP’s foot was tumbled and lost balance. In winter rainy season, the wet loess slope was very slippery. The IP didn’t observe the safety recommendation. The IP was lack of the safety awareness while working in hilly area. The crew was lack of supervision for the field operation. Lessons Learned : The shortcut will not be selected while working in the hilly area In winter rainy season walk slowly on the slippery slope. The safety regulation of the crew should be observed. The crew shall scout, design and build the route in advance. The group chief shall implement the responsibility of supervision.

Incidents and Analysis CountryClientDateClassification Iraq Kurdistan (8637B) DNOJan. 14, 2015Security Brief description of incident At about 08:50a.m. on Jan 14 th, 2015, a Toyota pickup for survey section was driving on a gravel road from camp to survey line with two pushers. The pickup was following on the right side of the gravel road. When the pickup was near station number 5113 on line 1137, the rear-right tyre was exploded by an anti-person mine. The driver reported to the crew immediately. At about 09:20 am, the mine clearance team arrived at the site and the de- miners cleared the site and led the vehicle to a safe area. In this incident, the rear-right tyre of the pickup was damaged. No person was injured.

12 2. Incidents and Analysis Causes: The anti-person mine was blasted by the running pickup. The mine on the access was not cleared by the de-miners. The mine clearance company could not detect this kind of anti-person mine. The client didn’t implement their promise to provide accurate mine distribution map of the prospecting area. Lessons Learned : The crew should negotiate with client to clear the mine in advance. The anti-person mine in access and lines shall be cleared. The crew should push the client to issue the accurate mine distribution map. The staff should receive adequate training about UXO knowledge.

Incidents and Analysis CountryClientDateClassification Sudan(Crew 9731)GNPOCJan. 10, 2015Environment Damage Brief description of incident At about 10:25 a.m. on Jan 10, 2015, one dozer operator in BGP Crew 9731 was opening the survey line in the bush area. He was attempted to keep the line as straight as possible. There were two trees on the line--the bigger one was in the middle of the line and the smaller one was on the side of the line, so the dozer operator remained the bigger one and cut the smaller one at the side of the line, which diameter was still about 25cm. One tree was cut, which diameter was about 25cm.

15 2. Incidents and Analysis Causes: The dozer operator didn’t observe the offset procedure. The dozer operator was lack of environment protection awareness. The field surveyor didn’t implement his responsibility of supervision. The crew didn’t give adequate environment training to key persons. The crew had no definite discipline for environment protection. Lessons Learned : The offset procedures shall be known by the dozer operators clearly. The environment awareness of the dozer operators shall be enhanced. The surveyor shall be enforced to implement his responsibility of supervision. The crew shall give adequate environment training to the key persons. The definite discipline for the environment protection shall be established.
