Expanded focus on Long-term Conditions (including the new Diabetes plan) 2016/17 National Planning workshop 13 Nov 2015

Long Term Conditions To support the implementation of the NZ Health Strategy, an ‘enhanced’ LTC section will be required in annual plans. Long term conditions are defined as ongoing, long term or recurring conditions, such as cardiovascular diseases, cancer, diabetes, chronic respiratory conditions, and mental ill-health etc that have a significant impact on people’s lives. Planning Objectives include having adaptive services to meet the increasing burden, providing people- centred care close to home, and to integrate services across all providers.

LTCs Annual Planning Guidance Overarching Goal: Reduce incidence and impacts of long term conditions through prevention, early identification and integrated management of risk factors. DHBs will need to describe an integrated performance story, demonstrating how DHBs will: Prevent and manage LTCs, reduce inequalities, reorient health services toward more co- ordinated support and support the workforce to develop skills for new models of care.

‘Living Well With Diabetes’ Vision: all New Zealanders with diabetes, or at high risk of developing type 2 diabetes, are living well and have access to high-quality, people-centred health services. Priorities include: Preventing people developing diabetes Detecting diabetes early and reducing complications Enabling effective self-management, providing integrated, quality care, and meeting the need of people with Type 1 diabetes.

Diabetes Annual Planning Guidance Goal: People living with diabetes are regarded as leading partners in their own care within systems that ensure they can manage their own condition effectively with appropriate support. Expectation that DHBs will implement the Living Well with Diabetes plan, and include actions for: Prevention and early intervention, reducing disparities in health outcomes, providing people- centred services, nationally consistent services, achieving effective self-management, and testing of promising interventions. Continue to progress the 20 quality standards for diabetes care
