10.  Paper Application: HAL Employee Cruise Request Form  Completed form must be sent to Human Resources Seattle  Once verified, then forwarded to Holland America Line Human Resources Benefit Cruise – How to Apply

11. Benefit Cruise – TIPS  If you request the $15 per day Benefit Cruise and get cleared 2 days prior, it will not change to a “Free” cruise.
